Sign up to receive 1095-C tax forms online

LEXINGTON, Ky. (Jan. 5, 2024) – In addition to W-2 tax forms, University of Kentucky employees can now elect to receive their 1095-C tax forms online. 

The 1095-C form shows evidence of employee health insurance coverage, in compliance with the Affordable Care Act. These forms are printed and mailed to employees each year for tax filing purposes. The IRS requires employers to mail these forms to employees unless employees are given the choice to opt out of a printed version. 

Online 1095-C forms will be available in Employee Self Service, similar to W-2 tax forms in late January. 

Under Employee Self Service, click on the “Benefits and Payment.” There, employees can find an option to elect an online 1095-C form. 

Once the online forms are generated in January, a “View Online 1095-C" link will also be visible in this section of Employee Self Service. Employees can then view or download a PDF of their form. 

If an employee does not elect to receive the 1095-C online, they will receive a printed version in the mail to their home address.
